hide details Jun 7 (1 day ago)
Summer greetings from the Information Superglacier™!
Our small community has been working away for over a decade to bring data communications, photonics (fiber optics) and technology jobs into the Finger Lakes Region of Western NY (the Information Superglacier™ also “Upstate”). Neither the problems (underemployment of an educated workforce) nor the solutions have been simple and the solutions that we have are just the beginning, not the end of our work. I’ve documented some great starts and a few “head scratching” losses over the last few years, but there seems to be a forward momentum infrastructure and advanced manufacturing, even if we have not identified the elusive “third prong” which will speed economic development.
Axcess Ontario recently announced that the 200-plus-mile fiber optic ring in Ontario County, N.Y., has been named among this year’s “CIO 100″ award winners by CIO magazine – the fiber ring’s first international recognition. Axcess Ontario is the public-benefit corporation that manages the fiber ring. The 24th annual CIO 100 awards program recognizes organizations around the world that exemplify the highest level of operational and strategic excellence in information technology (IT). Ed passed his own congratulations to Ontario County and Axcess Ontario.
Congratulations to Ed Hemminger! Recognition by Computerworld and CIO Magazine round out a great finish of public service for Ed as he enters the private sector with SMS Systems Maintenance Services. Thanks for putting the Information Superglacier™ (Finger Lakes Region) on the map through many hard fought battles and some outright serendipity! It took a while, but the State and the nation seem to be catching up! We are not resting on our laurels because we need to make sure that Axcess Ontario connects to several new broadband networks with roots in the Great Lakes and East Coast regions.
For Your convenience, Ron, I’ve put the full text of the press release at the bottom. I’ve added some text from the Axcess Ontario web site (after the release). Two Community Anchor Institutions have stories stories on the Axcess Ontario web site.
I am pushing to ensure that Axcess Ontario, the Southern Tier Network (who are named partners in the US UCAN BTOP application (but both of which were funded with non-Federal funds) gain access to the 8.8 Tbps US UCAN backbone. Despite all our efforts, our own high speed networks are still middle mile high speed networks. FTTH is still a distant glimmer. We DO HAVE cellular hybrid fiber / 4G wireless, but it may be a while before there is hybrid fiber / 4G wireless that does not follow current cellular pricing structures. (On the plus side, the hybrid fiber / 4G wireless may be FASTER than cable in the 700 / 800 MHz spectrum – Verizon & public safety spectrum).
We need better communication to let broadband development groups know that the non-Federal projects, which have sprung from the grass roots (private, State or Local funding) and previously funded Federal networks like the FCC Rural Health Care Pilot Program and several USDA Connectivity, Distance Learning and Telemedicine programs should be integrated and enhanced so that we can come up with a demonstrable way to reach the 26 million people NOT served in the rural communities and the underserved in every community!
The method to defeat the rural digital divide may begin with hybrid fiber / 4G wireless but that is only the first “jam cracker” to project the actual demand for services.
I am also stunned by the fact that leaders of Community Anchor Institutions (CAIs) may be unaware of their role in these new broadband networks for their benefits.
Just bringing broadband across the digital divide is not enough. I hope that these lengthy Information Superglacier (™) reports help illustrate that the broadband at the “middle mile” level is the best way to connect community anchors and attract the kinds of businesses that will bring last mile connectivity to end users. I hope this amalgam of some recent discussions is of use to communities in Illinois as several programs in Illinois, which seek to bring new broadband services and community adoption throughout the State. With luck, we can revisit the concept of the “Illinois Decade” if I get to Chicago by late Summer or Fall.
In April 2010, U.S. Senator Charles E. Schumer called on the Federal Communications Commission (FCC) to examine the success of the Ontario County fiber ring and consider it a national model as the FCC begins its push to bring ultra-high-speed broadband to 100 million homes by 2020. “The difference between economic success and failure in the digital world will be in large part determined by access to high-speed broadband,” Schumer said. “We must make sure that the groundbreaking achievements by Ontario County do not go ignored, but rather are spread to the surrounding areas and the rest of the state so we can attract business, foster high-tech companies and push the state’s economy into the 21st century.”
Axcess Ontario is a not-for-profit 501(c)(3) corporation established by Ontario County Local Law as a Local Development Corporation (LDC) – established on October 25, 2005. The LDC followed a grass roots initiative of citizens and Community Anchor Institutions (CAIs) with an intent to build, maintain and lease fiber-optic cable. I recently revived the community groups so that we can focus on the applications that make use of the Axcess Ontario network
The corporation is managed by a board of directors (9 individuals). All work is accomplished with contracts, including Chief Executive Officer, Chief Financial Officer, Legal Counsel, Construction, and Design and Engineering. The corporation is certified by the New York State Public Service Commission as a “Common Carrier” (i.e., phone company) and is New York State Sales and Use Tax-exempt. The corporation has completed an independent audit of all financial transactions in both 2007 and 2008.
State University of NY (SUNY) Chancellor Nancy Zimpher made a videoconference presentation at the STC in Canandaigua (“Developing the Talent Pipeline to Compete Globally, Grow Locally” Conference). She discussed the role of Community Colleges (CUNY) as community anchors for economic development. At the end of the presentation I asked the Chancellor about the role of SUNY, I2 and NYSERNET what the SUNY/CUNY role would be in connecting to US Unified Community Anchor Network (US UCAN). I also pointed out that I was standing next to the heads of Axcess Ontario, the Southern Tier Network, the Smart System Technology Commercialization Center of Excellence (STC CoE) and representatives of Cornell University NYSAES campus. The Chancellor said that she was not aware of the project but that someone at the presentation (in Canandaigua at the STC CoE) would get in touch with me (they didn’t).
If the leaders of the State CAIs are not aware of efforts to reach communities, then I fear the same can be said for local CAI leaders. Where does that lead the residents of communities for whom the middle mile and sustainable broadband efforts are supposed to benefit? There seems to be a disconnect in the chosen method of outreach and the intent of the legislation to bring broadband into rural communities.
We are fortunate that the Information Superglacier™ rests at the apex of a rural digital divide with a nadir within 50 miles. I suspect that Rochester (NY) has an urban digital divide which is also within 50 miles. Gaps in the Midwest can be hundreds of miles. When the solutions for urban and rural digital divides take place, there may be little similarity between the kinds of services used, but common issues in sustainable broadband adoption and the need for new infrastructure and reasonable price structures. The opportunity for tax exempt organizations and economic development groups to round out the universe of CAIs may be the least organized, but most resident oriented systems to help ensure sustainable adoption. Other CAI organizations also have specific tasks to accomplish.
The composite scope of what CAIs can and must do is mind numbing. I’m beginning to wonder if 8.8 Tbps will be enough, once these underserved communities begin to participate. Police and fire services can move from the “future” of P25 systems straight into 4G incident management systems. School and University networks can move to a K-20+ pipeline system that is more relevant to STEM (Science Technology Engineering and Math) and career development as part of the improvements in day to day educational communications. Hospitals and the surrounding medical communities can develop true healthcare information exchanges, telemedicine and PACs/DICOM networks.
Although I’m not an engineer, I’m deeply steeped into the technology. When I speak about the urgency of connectivity, I see eyes start to glaze over or (worse) start to look in panic for the nearest exit. It is not easy to bring infrastructure to a community that does not have it. In part because new technology creates fear uncertainty and doubt. This also means a leapfrog of technology from pre-Internet to the fastest systems on the planet. Some of the same people who fought the idea of 4G technology in 2005 are cruising the net on 4G LTE or WiMax, today.
When I push to bring new technology without a cellular carrier or as a lead to FTTH, I will experience similar resistance. Early adopters have a lonely road on the Information Superglacier™, but in the last few years the presence of Middle Mile broadband has allowed us to fill the STC CoE with new tenants and push for the development of a stand alone data center on the STC CoE campus (largely because data centers compete with clean room facilities – in building, but can lead to better electrical infrastructure for all parties as a stand alone.
In short, the elimination of the digital divide in rural AND urban communities can represent an exponential shift in the use of broadband over a meager baseline.
It can’t be done without infrastructure and it can’t be done, well, without the CAI leaders knowing that there are new tools available to help those leaders advance. The integration of broadband technology must be integrated with economic development and education. Our communities face a significant SHORTAGE of workers with the right skill sets for advanced manufacturing jobs in the region.
Chancellor Nancy Zimpher had her work cut out for her. On the same day that I was asking about the role of the SUNY/CUNY system in bringing US UCAN to Western NY, Mike Mandina, head of Finger Lakes Advanced Manufacturers’ Enterprise (FAME) had a few questions of his own. The videoconference portion was not recorded and I did not see a press release or transcript. Here is a clip from a recent FAME newsletter.
FAME e-News – May/June 2011
· Mike Mandina (FAME Chair and President of Optimax Systems Inc.)
This past month I was honored to participate on a panel hosted by Ontario County that included SUNY Chancellor Nancy Zimpher. The topic was “Developing the Talent Pipeline to Compete Globally, Grow Locally.” Our three area community colleges were present and the event was hosted at the Smart Systems Technology and Commercialization Center of Excellence (STC), (formerly Infotonics). Having an opportunity to speak on behalf of Advanced Manufacturers to someone as influential as Chancellor Zimpher is a rare occasion and I did my best to strengthen her awareness of our skilled worker shortage.
The recent report “Skills2Compete-New York Campaign” projects 996,190 middle skill job openings statewide through 2018. In our 9-county region alone, officials project 47,700 middle-skill job openings over the next 7 years, in addition to 47,500 high-skilled job openings. The Chancellor mentioned that she has top business leaders as part of her advisory group, she mentioned Fortune 100 firms– among them Pfizer and Xerox. I seized the opportunity to ask her if I should be concerned about her awareness of the needs of the advanced manufacturing companies that make up the FAME membership. We are small to mid-sized firms who are growing and hiring students from the SUNY system.
I further referenced the GRE Eyes on the Future event that took place the prior week, in which the Chancellor was a panel member. On the panel she was accompanied by the President of Kodak’s Film, Photofinishing and Entertainment Group. My point was that Kodak had not increased net employment in Rochester in the past 20 years, and the same is true of Xerox, and yet Kodak was the sole manufacturing firm on the panel. So, I asked if FAME members should be concerned that she may not be getting input from the firms that are most likely to hire SUNY graduates. She assured me that I should not be concerned, but I am. I believe she was listening, but I will be repeating the message!
Thank you,
Mike Mandina
I am 100% behind FAME’s efforts to bring STEM to the schools through education, Lego & Robotics programs, the new student tours initiative and more. They have also been sponsors of Sam Samanta’s new program at the Victor FLCC Campus. FLCC and Thompson Health were among the first to connect to the new fiber optic network. In turn, FAME and the optic cluster have made this region an influential powerhouse in the development of new fiber optic systems to accelerate that which we already think of as fast, because fiber optics is a dynamic market (even if it sometimes gets ahead of itself). The Finger Lakes Community College Associate in Applied Science Degree, A.A.S.
Finger Lakes Community College offers a two-year program leading to the Associate in Applied Science in Instrumentation and Control Technologies degree. This program provides graduates with multidisciplinary expertise to address workplace demands of emerging technology based businesses, by providing basic proficiencies for emerging knowledge and skill areas in data acquisition, automation, mechatronics, and control systems.
The tight integration of the courses in mathematics, physics, computational skills; in turn are leveraged in subsequent courses in electronics, design, process improvement, data acquisition and automation technologies; all in a context of active learning methodologies such as project based hands-on problem solving, case studies, and reinforced through job shadowing and required internships. Soft skills such as communication (written, verbal, presentation) and teamwork are integral part of each of the technical courses in the program.
I can’t think of a reason to develop a high technology network unless it is for the purpose of economic development. The problem with the networks that provide those services is that it is not the network (or other infrastructure) that provides the economic benefit, it is the transport and delivery mechanism. The other “prong” (besides the gamut of infrastructure) is advanced manufacturing, but the problems is that the economic development for advanced manufacturing tends to produce fewer (but better paying) jobs than traditional manufacturing. I tend to think of broadband infrastructure in the same way that I would think of a road, bridge, electrical service, etc.
No one would dream of leaving 26 million people without infrastructure services, but Chuck’s original message of having 26 million Americans cut off AFTER the investment of over $7 billion dollars.
The number of round one proposals for the first $4 billion was for $28B in new projects with $10B in private investment. It is too bad we did not have a way to organize and fund the runners up with the private dollars. It took billion dollar Google almost as long as the BIP/BTOP program to pick ONE community for Fiber to the Home. Neither the pace of the government, nor the pace of private sector is anywhere near the rate of uptake within other companies that are leaving the country that invented Internet technology in the dust.
On the plus side, I have met many forward thinkers in the BIP/BTOP design phase and those who did not secure methods of Federal fund immediately began to search for other solutions. Axcess Ontario was already in gear, Southern Tier went forward with a mixture of funds, Ramsey County is moving forward and adding a Tier III+ data center. The benefit of a good design proposal is that it can be shown to other investors. When it was clear that Axcess Ontario would languish in a Federal State battle, we just opted with local funding and a $5 M solution popped up and covered an initial shortfall in the total funding package. As soon as the funding was secure, Axcess Ontario became attractive to a variety of commercial prospects. We are still far short of a last mile solution, but every area in the county should be reachable by hybrid fiber / 4G wireless.
Press release:
Canandaigua, N.Y., June 1, 2011 – Axcess Ontario is pleased to announce that the 200-plus-mile fiber optic ring in Ontario County, N.Y., has been named among this year’s “CIO 100″ award winners by CIO magazine – the fiber ring’s first international recognition. Axcess Ontario is the public-benefit corporation that manages the fiber ring.
The 24th annual CIO 100 awards program recognizes organizations around the world that exemplify the highest level of operational and strategic excellence in information technology (IT). Other award winners this year include Advanced Micro Devices, Dell, DreamWorks Animation SKG, IBM, and GE Energy. Only a limited number of public-sector award winners were named in addition to Ontario County, N.Y., including Chicago Public Schools, Fairfax County Public Schools in Northern Virginia, and the U.S. Marine Corps’ Global Combat Support System/Logistics Chain Management.
“The CIO 100 award is one of the most important recognitions that Axcess Ontario has received in that it ranks the Ontario County fiber ring among the top IT innovators internationally, significantly reinforcing our position as a broadband model for the United States,” said Ed Hemminger, CEO of Axcess Ontario. “In addition, the magazine reaches the top information technology officers and executives around the world, a key audience as we work to add telecom carriers and expand business and residential services using our fiber ring.”
The CIO 100 award is the third major award that Axcess Ontario has received in the past 10 months. The CIO 100 award comes on the heels of last month’s recognition by Computerworld magazine as a 2011 Honors Laureate. Axcess Ontario was also honored last fall by Harvard University’s Ash Center for Democratic Governance and Innovation at the John F. Kennedy School of Government as a “Bright Idea” – a promising, innovative solution that can assist other communities as they face their own challenges.
Axcess Ontario board member David R. Smith, chairman and CEO of Plures Technologies Inc. and chairman of SensiVida Medical Technologies Inc., nominated the fiber ring for the CIO 100 award.
“The people of Ontario County have every reason to be proud of the fiber ring project and the Axcess Ontario team,” Smith said. “Thanks to the county’s leadership and the public’s support of the project more than eight years ago, Ontario County is now recognized as one of the top IT communities in the country and around the world.”
The 2011 CIO 100 awards will be featured online at www.cio.com on Aug. 1, and in the Aug. 1 issue of CIO magazine. The awards will be presented at The Broadmoor in Colorado Springs, Colo., on Aug. 14-16 at the annual CIO 100 Symposium® and Awards Ceremony.
About Axcess Ontario
Imagine where fiber technology can take us. Axcess Ontario has. With an open-access, dark-fiber infrastructure and a non-profit approach – serving as an example for a national model – the 200-plus-mile fiber ring has Ontario County, N.Y., on the fast track to greater economic development, better quality of life, and global competitiveness. By providing this critical source for advanced connectivity and increased flexibility through quicker, more affordable broadband, businesses and organizations, as well as residents, will have improved capabilities like never before.
From the site:
———————–
Finger Lakes Community College (FLCC) tapped into the Axcess Ontario fiber-optic ring in the fall of 2009, immediately increasing its Internet access bandwidth from 40 to 100Mbps, a 250% increase in capacity.
When FLCC’s Victor campus center opened in January 2010, the college installed a 1Gbps secure private network connection to the main campus, meaning access to the network at the Victor campus was just as fast as it was at the main campus. The expanded service also enabled the phone system to transfer calls easily from the main campus to Victor, and vice versa. FLCC plans to connect its campus centers in Newark and in Geneva to achieve improved backup systems and educational programming.
“Axcess Ontario’s fiber-optic ring means FLCC can save money and improve educational experiences for students and faculty by providing centralized computer services and Internet access to all of its locations through the main campus,” said Rick Evans, director of information technology at FLCC. “By connecting its main campus and campus centers to the ring, Finger Lakes Community College will be able to build a wide area network that allows for videoconferencing and state-of-the-art services across all locations.”
———————–
In the fall of 2009, Thompson Health signed on to the Ontario County fiber ring and began using a larger bandwidth connection between its main campus at F.F. Thompson Hospital in Canandaigua and the Urgent Care Center based at Thompson Medical Center in Farmington.
This connection enables the transmission of diagnostic images between the facilities as part of the digital X-ray services they began providing at the Urgent Care Center in November 2009.
“For our patients, this connectivity means that diagnostic images taken in Farmington can be immediately reviewed in real time by radiologists at the hospital more than nine miles away in Canandaigua,” said Mark Halladay, director of information technology for Thompson Health. “For our organization, the connection also provides another advantage-large bandwidth at reasonable cost.”
In 2010 alone, the X-ray department at the Urgent Care Center performed 3,237 X-ray exams. All exams were read by radiologists at the hospital.

Friends of Mel Bergstein Facebook Page and kind words of support
[Editor's note: Ron May here. I found out from David Moon who goes back to the mid-1980s with Mel Bergstein that Mel is in the hospital in New York at Sloan Kettering. David and Michael Krauss were at the 7:30am networking event prior to the breakfast at the Executives' Club on June 1st. Mel is quite ill and while David did not tell me what Mel's health problem is, Sloan is known for cancer. David Moon (davidmoon521@gmail.com) also told me that there is a Friends of Mel page on Facebook and I asked Melanie to look it up. Melanie sent the following information. Moon did tell me a funny story about Mel from the 1980s. I'll tell that next time. OK, I'd better tell it now or I'll get off on other things. David goes back to the days when he and Mel were with Andersen. David asked a personal favor of Mel. He asked Mel to go to Florida to speak on some C-level panel. Mel went and gave the talk. Dave's father-in-law and mother-in-law were there and they were swooning over Mel, whom many people have told me is charismatic. So when David went to see his in-laws, all they could do for 45 minutes was talk about how great Mel was (as Dave told the story it was clear to me that over the years he has gotten used to and tired of all the adulation for Mel) along the lines of yada, yada, yada. When David saw Mel, he did tell him that his in-laws had gone on for 45 minutes about how great Mel was. Mel's reaction: "What, only 45 minutes?!" And that pretty much captures the teeny weeny ego of Mel Bergstein. One of these days we need an informed article on whose ego is bigger: Mel Bergstein, Bill Weaver, Howard Tullman, Mel's former partner Al Beedie -- or is that Beadie? -- Flip, the late Bob Bernard -- who gets top honors for that one?]

I know my father usually posts here Friday night, but this weekend he won’t be, and we just wanted to let those in the group know why.
The reason is that over the last few days he has been suffering from an infection. The good news is it now seems controlled, and he is resting comfortably in MSKCC’s intensive care unit.
He welcomes all of your thoughts and good wishes, but it is unlikely that he will post for a while.

So, now to the transplant itself. I’m sorry if the picture is a bit graphic for some of us. But, it is what it is. The transplant itself is pretty much a non event. It’s just an infusion of several ounces of fluid into basically an IV line. Takes 10 minutes. People do have reactions in extreme cases. So, there’s lots of monitoring. But, at least in my case, a pretty tame event.
However, it is quite symbolic as you can imagine. It’s hopefully a curative event for a very tough disease.
As Tina, the boys and I were waiting for the doctor and the cells to arrive, the nurse who was preparing the room sticks her head in to ask us if the lemons had arrived. Lemons? Didn’t we just do rabbits? I asked the nurse if there was a monkey on the way. Then I turned to Tina and Seth (Adam was still on his way in a cab) and said, “is this getting a bit like witchcraft?” And then we had 10 minutes of howling, screeching laughter. I guess you had to be there. As it turns out, the lemons are only necessary in certain cases where the donor’s cells are treated with preservatives. The monkey was a figment of my imagination. The rabbits of course, had already kicked my butt. But, it was a moment of levity for all of us.
Perhaps the most meaningful moment came during the infusion. The doctor who did the infusion runs all the transplant activities at MSK. He is a leading bone marrow transplant clinician and researcher with world wide credentials; a Venezuelan by birth. In the middle of the infusion, he uttered a few words under his breath but clearly in earshot of all of us. I asked what he had said.
His answer was: “The words that I always say when I infuse cells is, Obre Dios y que se logre.” In English, “Let God do his job and it will succeed.” I was deeply touched that an accomplished man of science would take the time to recognize the presence of God at that moment.
Perhaps the world would be a better place if we all had the humility to recognize the presence of God in what we do. And perhaps, while recognizing the presence of God, we could recognize our common humanity.
Tina and I appreciate your continuing support. We’re doing our marathon a day at a time, but we’re hoping we’ve turned the home leg.

Thanks for all the good wishes, prayers and supportive comments. Also, thanks for the recommendations of audio books to read. Tina and I take great comfort and encouragement in all your support.
I’m sorry I haven’t been on the air lately. The good news is, CHEMO is OVER!!!!! Twelve days of treatment, nine as an in-patient. The last couple of days have been difficult. It’s not quite the “Bucket List”, if you’ve seen that movie. MSK has made great strides in treatment and the doctors and nurses take a very compassionate approach. They anticipate the onset of side effects very well and treat them vigorously; great people who are totally committed and know what they are doing.
I have explained to every nurse that a “shank” is a very bad golf shot. And, golfers do not use that word on the golf course, because, as soon as someone utters the word, someone else hits one. So, I’ve told them not to use the “v” word in my room. So far, so good.
As bizarre as it sounds, one of the final chemo treatments involves being injected with rabbit (yes, I said rabbit) anti bodies. These are sort of the advanced guard to beat down my defenses to the donor’s stem cells. My body did not like the advanced guard very well and the rabbit kicked by butt; which it was supposed to. But was not fun. Now, my nose is twitching and I have this insatiable desire for cabbage. I’m told I’ll get over it.
The good news is that chemo is over and this afternoon we get the stem cell transplant; thanks to an unpaid, unknown donor who took the time to register in the marrow bank, to be examined and qualified and to donate. The world needs more donors.
Anyway, we’re moving to the next phase. This is the building back phase. It’s the next leg of the marathon, which we’ll do one day at a time. Thanks again for all your support.

Your status reports let us all know that your spirits are high and that you continue to have a marvelous sense of humor. So, a brief story. When I was on the 15th floor my roommate and I often walked the 14 laps equals a mile hallway together. We dutifully pushed our IV poles. On one such walk we observed a woman bobbing and twisting in an effort to untangle herself from her IV tubes. With out missing a beat my roommate said, “Oh, look we have a pole dancer for our entertainment today.” She broke out into a big laugh and asked if he would help her out of her predicament. He doubled back to the nurses station and returned with an aide who straightened things out. He went home the next day.
Yup, a sense of humor and laughter are good for ones well being.

Glad to know that you are attacking this with all the gusto (and sense of perspective and even humor) that you’ve applied in every other venture of your life. FYI, I know one other person treated at MSK for leukemia – he is at the other end of the age spectrum (5 yrs old), but very complicated case. Other medical centers wouldn’t or couldn’t even take the case. He’s doing fine today, 2 yrs later. That will be you, buddy, because MSK is the best!

So, as you arise on this beautiful Spring morning, you are probably wondering what is happening at the poop palace. Perhaps not. But, here goes anyway.
The transplant poop palace is considerable nicer that the old poop palace. The reason is that the transplant patients are in isolation; i.e., no going out of the room for the entire in-patient stay. And, every room is a solo. So, the rooms are bigger, maybe 28′ by 30′. I have a window over looking York Avenue. The bathroom is large and functional; so all in, not terrible.
Like last time, I am hooked up to a chemo pole. Except this chemo pole looks like the Rockefeller Center Christmas tree. The old chemo pole, by comparison was twig. The pace on the transplant floor seems more frenetic and more precise; hanging new meds, taking them down, giving pills, etc.
Chemo poles are powered by electricity through a power chord. Of course, the chords are not quite long enough to make it across the entire room. So, one is constantly plugging and unplugging the poles. Seems like there might be a better technology answer to this power problem. On the poles are pumps which are connected to tubes dripping toxic and non toxic fluids into your veins. Seems to me these pumps could use a little technology work as well. They have audible alarms when the flow isn’t correct and the nurses seem to have a lot of manual work to do to get the pumps programmed properly.
One of the keys to success I’m told by all the medical staff here is to stay as active as possible. All this activity has to happen in the confines of one room (plus bathroom), attached to a short leashed, power hungry chemo pole while the chemicals are dripping.
All this seems to beg for a bit of systems design. I suspect however, that what it really begs for is capital and real estate. Critical cancer care in the inner city is not easy. But, this is where some of the best medical staff in the world are. So, they put up with the constraints.
But, I’m learning the ropes. One thing I’m learning is that Ativan is the duck tape of the medical profession; at least here at MSK. The nurses are very sparing with the dosages, but it does help with many things including nausea and anxiety; lots of which exist here
The nurses came by today to give me a bit of a warning about that the next few days might bring. I told them that we never use the work shank on the golf course because when you say the word “shank”, somebody hits one. Therefore we agreed never to use the “v” word in my room.
Tina and I appreciate your support. We’re doing a marathon one day at a time.

I hope everyone had a good Mother’s Day. We spent it with Seth, Alex and the grandchildren; a special time for us.
Today is the day we’re going into MSK to start the transplant process. There is no question that the prospect is a bit daunting. But, while there is material risk, it’s a choice for life. I spoke about going forward with Seth and Adam individually last night and of course, Tina and I are always talking. We all agree that the planets have lined up in a way that’s unlikely to happen again; i.e., we have a 10×10 donor, my leukemia is in remission, I’m not getting any younger and I made it though a very grueling physical screening to get here. And of course, we have some of the best docs on the planet caring for us.
So, we’re moving ahead. We understand that one of the keys to success is, while in isolation, to stay connected to the “outside” world. That’s where you come in. You are making a difference for Tina and me. I’ll do the best I can to keep you posted. Thanks again for all your support.

Tina and I appreciate all the kind thoughts and encouragement we’ve received over the last couple of days. So, here’s just a quick update. The nurses and patient care on the transplant floor continue to be great just as they were on the 12th floor at MSK; super competent and caring. On the treatment, so far, so good; but my guess is we’re not into the teeth of it yet.
The accommodations are significantly better because this is isolation; no leaving the room. While we thought visitors were a good idea, we’re now learning no so much. Which actually makes sense. But, I have a window overlooking York Avenue (East facing) and so I get the morning sun.
We have an apartment for Tina just a few blocks from the hospital and the weather, thank heaven, is finally good; so she can walk back and forth. So far, that seems to be working well.
I wish I had something more amusing to offer you. I’ll work on it. Meantime, we’ll take this marathon one day at a time. Thanks again for your support.

Tina — Happy Birthday! Here’s wishing that the start of Mel’s transplant treatment turns into the best present possible for you both, and for your family. If there’s one thing that I could change about this group, it is that we would have named it “Friends of Mel and Tina” because, as you know, we love you both and wish the best for both of you.
